The registration of birth is done in the district in which the birth occurred and by the following persons: the father/mother and or both parents, the next of kin of either parents, occupier of the premises which the child was born, an adult present at birth or a care taker of the child. PROCEDURES AND REQUIREMENTS Parent of the child must go to the nearest Birth and Death Office in the District where the birth(s) occurred Present evidence of birth(s) (i.e. maternal health record book) Provide bio-data to the birth event Name of child Date of birth (would have already been indicated in the maternal health record book) Place of birth (would have already been indicated in the maternal health record book) Father’s name, age, occupation, religion, nationality and educational background Mother’s name, age, occupation, religion, nationality and educational background
